Scoreboard - Stats to Discord?

Im looking for someone which may help me with current idea.

I would like to get current data to discord from our Gather server.

Like after map end there will be stats for each player:

Top Player
Kills
Deaths
Map

Winning Team: CT 16:5 T

Like this but on discord -> SQL Matches - AlliedModders
